I have been curious about this bug as it has persisted for several
releases. Adding some info here. I am using Ubuntu 12.04 and haven't yet
tested 12.10 to see if the behavior is the same.

To quickly see the lack of icons in Help --

Open a LibreOffice application, and choose Help --> LibreOffice Help

The LibreOffice Help page that appears should have two icons on it.
(Pressing F1 takes you to a different page depending upon which
application you are in, and none of the application-specific home pages
have icons on them, though there are pages within them that do.) The
icons vary depending upon which style is chosen.

I have noticed that when I open Tools --> Options... --> LibreOffice --> View
the default Icon Style says "Automatic (Tango)" and the icons don't appear in 
Help.

HOWEVER I find if I choose either Tango or Human styles, the icons
appear in Help, and the icons persist even if I switch back to Automatic
(Tango) until the next time I close and reopen the LibreOffice
application.
