Without further details it is difficult to come to a conclusion. That
said, failing hardware could cause /etc/shadow to be unreadable or
contain garbage. Failing memory could cause the password hashing to
fail.
Perhaps some application has modified the pam settings in /etc/pam.d/ to
require different authentication methods.
But without further details it's all just guessing.
