Mmmm...., gents, I was looking at the last screenshot of my output
settings and had a hunch.  I decided to move the output volume to about
75% and still had no sound.  I moved it to about 80% and still no sound.
I moved it to 100% and bingo, there was sound!  Why would there be no
sound at levels lower than 100%, I have no idea.  I actually tried it
twice, and got the same result on both occasions.

I then went back and removed the ";" from the "required-any" lines, restarted 
pulse audio, and I had sound.  So it seems that the problem was with the audio 
levels being less than at maximum.  That doesn't make any sense to me seeing 
that at that level the sound was so loud that I had to quickly mute it.
Maybe something is a bit buggy but at least I have sound on the laptop.
