I'm using Ubuntu 12.04.1 with gnome-shell.
After the machine is inactive for sometime and the screen gets automatically 
turned off and locked,
when I move the mouse or hit a key to wake it up most of the times the screen 
turns on, I see the desktop's background,
and no password prompt, and the desktop is unusable (I can't do nothing).

After suffering from this problem for a while the only solution I found is to:
1. Ctrl-Alt F1 to go to a console terminal
2. log-in and do killall gnome-screensaver
3. Go back to the graphical terminal (Alt+F7), now gnome is still half 
functional (I don't have the upper panel), so what I do is: lock the screen 
with Alt+Ctrl+L and then enter the password and I get back to a function 
desktop.

I've seen this problem on at least three different machines, both with a
single monitor, and with 2 monitors.

I'm having this problem for more than 6 months, I hope it'll be gone after 
every update,
but it's time to file a bug report (:
