I realize this bug is old but I can confirm it's still happening to me (Ubuntu 12.10 running on a MacBook Pro). Restarting pulseaudio (pulseaudio -k) fixes the issue... until the next time it happens :) A little annoying, but I guess it must be hard to fix do to the 'random' nature of it...
