I believe I had the same issue when upgrading from 11.10 -> 12.04 on a LUKS -> LVM setup. I believe my issue was caused by a superflous lvm=/dev/... for each device in /etc/crypttab. That seems to have been picked up by update-initramfs and caused funny entries (with two "lvm=...") in (initrd)/conf/conf.d/cryptroot.
I had picked those entries in /etc/crypttab up from some (old) tutorial from the web.
