** Description changed:

  Hello,

  I've been able to replicate this issue with two different wireless
  cards, a PCI and a USB.

  The PCI card is: Rosewill N180PCe
(http://www.newegg.com/Product/Product.aspx?Item=N82E16833166063)
  And the USB is: Negear WG111 v2

  I've attached the following files:
  * Output of /var/log/syslog when the error happens
  * version.log
  * lspci-vnvn.log

  When the issue happens, I'm disconnected from the AP, the password box
  pop-ups for the AP, no matter how many times I re-enter the password it
  will not connect, and the only way I can solve this is to restart the
  machine.

  This happened in both 12.04 and 12.10 (I upgraded hoping to fix this
  issue).

  Description:  Ubuntu 12.10
  Release:      12.10

  You can see ~2m prior there was a successful "rekey", and then the 'No probe
response' and after that I lose all connectivity and the password prompt
appears.
