I confirm that this bug is fixed by the solution presented in comment
50.

I am using ubuntu 12.04 64bit with the latest updates and confirm this
exact issue with a standard wired usb mouse. The keyboard is also
affected but with a much lesser frequency than the mouse.

Since applying the fix in comment 50, I have had no problems whatsoever.
I immediately noticed this problem when recently upgrading to 12.04
after years of trouble free operation on the previous ubuntu LTS
release.

Based on my understanding of how the fix works by unblacklisting the
usbmouse module and the intermittent nature of the problem (eg 50%
failure rate on boot) - I would say that the hid handling subsystem of
the 3.2.0-37-generic kernel has some kind of race condition which causes
it to fail to identify the usb mouse.

In my 15 years of using linux this is the first time I've had a problem
with a mouse being recognised!
