A little hint on this - if I turn off "Menus have icons" in Ubuntu Tweak tool and restart the application (tested Geany and Shutter) - the duplicate menu items are no more, or to be correct they now have correct labels.
Which package should this belong to, hints anyone? I'm changing over to unity for now. Running Raring now, so this still affects newer versions.
