I used Startup Disk Creator to erase and re-create an installer USB
stick and it also failed on the second boot (and subsequent boots). This
time the error was more clear:

General error mounting filesystems.
A maintenance shell will now be started.
CONTROL-D will terminate this shell and reboot the system.
bash: groups: command not found
root@ubuntu-gnome:~#

Ctrl+D just reboots.
