KDE - Ubuntu 12.04 LTS
AMD 64 Athlon 3500
Quadro FX 1400 PCIe video card
1) Fresh install -- all AOK
2) Upgrade from 3.2.0-37-generic to 3.2.0-40-generic.
3) Reboot -- GRUB loads then screen briefly flashes "No Signal" then black
screen-o-death.
5) Booting to previous version works fine.
