I noticed this error on Ubuntu 13.04 beta, using an own desktop environment. (maybe it affects unity, but it uses a different titlebar for maximised windows, might be hard to notice)
This script launches my environment: #!/bin/bash gnome-session & compiz ccp & nautilus & synapse & tint2 Symptoms: If I open a Nautilus and for example Thunderbird (both fullscreen) one of them loses it's focus when it's titlebar is clicked, and it switches to the other window. (So if you want to close a window, you end up closing another.) The windows get the focus when I click on them, but clicking the titlebar loses it. After minimizing windows an restoring them, the problem is gone. Sometimes both windows are "unfocusable" by clicking on the titlebar. Sometimes one window is acting like it's "always in foreground".
