I have the same problem. Restarting pulseaudio or reload of alsa (as suggested) does not help. Restarting make it works again, but only for a short while until it stops (again). I've tested with the "stock" 13.04 x64 kernel, and 3.9. No difference. Audio worked fine on the 12.10 system I upgraded from.
