1) Identical machine, same Ubuntu fresh installed from the same media CD, but with a PS2 mouse instead of a USB mouse, is not suffering this. 2) Randomly happening with the machine online (even spare). I get this logged to dmesg, like if the mouse were being disconnected/connected. And this one worked perfectly. The only difference is that the port I plugged in the new one was 3.0, while the crashed one is 2.0. Will test now by having my crashed mouse on a 3.0 USB from boot time - If it doesn't crash, then this problem may be affecting only USB 2.0. Any ideas?
