Public bug reported: System: Ubuntu 13.04 on Sandy Bridge x64 Dell laptop.
The program "startup disk creator" failed to create a working USB stick device. Boot error: "Operating System Not Found". After sudo dd if=/dev/sdX1 of=~/usb-stick.img bs=10M sudo dd of=/dev/sdX if=~/usb-stick.img bs=10M sudo rm ~/usb-stick.img (solution found here: http://wiki.ubuntuusers.de/Live-USB#Problemloesungen) the system was able to boot. Next error was : "An attempt to configure apt to install additional packages from the CD failed." See same Bug #658865 After "sudo rm /usr/lib/ubiquity/apt-setup/generators/40cdrom" installation worked so far. Please fix the program Startup Disk Creator. Its not working for a really long time now. Even Unetbootin does not work properly.
