Can you check with a live CD of a newer Ubuntu version (13.04/Raring or
13.10/Saucy) whether your problem persists there?

Can you also follow the steps in

"USB printer", "CUPS error_log", and "Capturing print job data" for a
failing case?

  Canon iP4000 printer stopped working

Status in “cups” package in Ubuntu:
  Incomplete

Bug description:
  After upgrading to 12.04, the printer worked for a while, stopped
  working, then worked again after testing with windows xp and/or
  running the steps in https://wiki.ubuntu.com/DebuggingPrintingProblems
  (results of running these steps are below).

  Now the printer doesn't work and I can't seem to revive it.

  Neither the driver downloaded from the system tools directory nor the
  cups driver seems to work now.
