Last command gave no output.
And, yes, it's 100% reproducible. I'm using a Nexus 4 on T-Mobile. I
must be in or travelling through an area with poor signal everyday,
because everytime I want to use internet on my phone, I have to reboot.
I have not once in some weeks or months seen it reconnect after that 3G
icon has gone.
