OK, I purged cups (removing /etc/cups/ppd) and re-installed from
scratch. This fixed the problem. I had copied the /etc/cups/ppd
directory and the /etc/cups/printers.conf from a previous install so I
guess that the problem was in one of those config files.

Incidentally on another machine I had a similar problem where acroread
only printed the first page. This turned out to be because I needed to
set acroread's "Print / Advanced / Font and resource policy" to Send
with each Page. The same fix didn't help with this problem but I thought
this might help desperate searchers!
