I have a IOGEAR Bluetooth 4.0 USB Micro Adapter (GBU521) which cannot
audio sink with my bluetooth headphones (Kinovio BTH-220).  Headphones
can be detected and pair, but audio sink or mic input doesn't work.
Blueman manager gives the error "Connection Failed: Stream Setup Failed.
It used to work in the LTS Ubuntu version 12.04 but now no longer does
with a kernel update.   This Broadcom chip BCM20702A0 has had many bug
reports on it on launchpad which have been fixed in previous releases.
There is a lot of info in these previous bug reports that is useful for
people diagnosing and solving the problem, although I did not understand
most of it.  From reading them, I believe the problem comes down to
vendor ID support or the issue of the adapter needing to load a special
firmware.
