This is an issue found in 12.04.3 with latest 
upgrade(linux-image-3.8.0-35-generic ),
After user removed SD card by "Safely remove" option, the card reader won't 
work anytmore until  system rebooted.
It's also verified with "linux-image-3.11.0-15-generic", having the same 
problem.

Steps to reproduce:

0. Insert the SD card first, make sure it works.

1. Right click on the USB icon on the launcher
2. Click "Safely remove"
3. Re-Insert the SD card that works before

Or
1. Open "Files" or "Home folder"
2. Right click on USB icon located at left side below "Devices" list
3. Click "Safely Remove Drive"
4. Re-insert the SD card that works before

Actual result:
Nothing happen, card reader is silent

Expected result:
the USB icon should show up, or popup a window to open the SD card
