I've tracked it down to a shutdown() call in audio/headset.c so far. It
seems to fail with ECONNABORTED on the mako; so nothing carries through
with the HUP expected to happen when the SCO socket is really closed.

On maguro, the socket appears to be properly closed (which may well
indicate a firmware issue), however I still cannot properly make use of
the headset, it will just loop back sound from the microphone to the
speaker.
