This particular bug was introduced intentionally, as a fix for Bug#447788, which has a lot to do with why nothing has been done about it. The engineer responsible at the time was of the opinion that a web site with a case sensitive username was broken, so would not fix this. I may or may not agree, but I am very much of the opinion that it is not up to the browser to decide that, and as there ARE quite a number of web site's like this, the password manager should be able to deal with it.
