I used Pen Drive Linux's USB Installer on Windows to create the bootable installation flash drive (using its Ubuntu server option). I have followed the instructions on this page: http://www.ubuntu.com/download/desktop/create-a-usb-stick-on-windows
I wrote several trusty server installer version onto the same flash drive. Somehow that worked yesterday, but not today. Today all of the trusty versions I tried had the problem described in this issue. Now finally, I used Startup Disk Creator on Ubuntu 14.04 to create the flash drive, and that resulted in a working installation.
