That definitely looks like a notify-OSD fallback. I'm wondering if the
indicator-network secret agent is running instead of the one coming from
nm-applet.
Next time you see this, while the dialog is open, could you do this:
$ ps aux > ps_aux.txt
and attach the ps_aux.txt to this bug report. (just make sure it does not
contain anything you don't want to be public).
