Is this a new installation of entire system, or was user home directory preserved?
In case this is reproducible, it would be useful to capture standard output & standard error of unity-lens-applications, e.g. kill existing process and start a new one from a terminal like this: /usr/bin/unity-scope-loader applications/applications.scope applications/scopes.scope commands.scope But I realise it can be challenging if terminal cannot be launched...
