Public bug reported:

On Ubuntu GNOME 14.04 with apt 1.0.x, apt-get exits even though I press Y when 
prompted. This happened to me rarely, maybe 1 time out of 20, but I'm not sure. 
It may be a random behaviour.
Unfortunately, there's no log of this faulty behaviour in 
/var/log/apt/term.log, but here's an example:

sudo apt-get dist-upgrade
...
...
...
Do you want to continue? [Y/n] y
exiting.

My system uses Italian language, so these might not be the exac translation.
