I just had this happen to me after un-docking and then re-docking my
laptop. When docked, it's connected to two monitors (laptop screen not
used), and when not docked, it reverts to just the laptop screen. Had
to restart the OS (Ubuntu 14.04 64-bit) to get the alt-tab menu and the
unity bar out from behind every window and back to the front.
