I've also encountered this issue with a dual monitor setup. I only seem to trigger it when locking my Dell Latitude E6530 (NVIDIA Corporation GF108GLM [NVS 5200M]) with Ubuntu 14.04.1. Sadly I cannot reproduce it reliably, but I trigger it in maybe 5% of the cases.
