same issue here. Have not been able to pinpoint it. I have it on 1 machine, on 
a few others there is not issue.
I tried reinstalling the ubuntu-desktop package, network-manager and 
network-manager-gnome with no results.
This is on a fully updated 14.04.
My output:

nm-applet --shell-debug
nm-applet-Message: using fallback from indicator to GtkStatusIcon

(nm-applet:2865): nm-applet-WARNING **: Could not find ShellVersion
property on org.gnome.Shell after 5 tries

When I run that command on a working machine, I just get a second applet, but 
the warning message is something with nm-applet:3420.
Is this a different nm-applet version?
When I do a file /usr/bin/nm-applet I get the same sha1 on both machines, so it 
seems not.
Pretty clueless, hope this can be fixed.
