I apologize for your bug not getting a response until now. The error you are
seeing appears to be the result of a bad disk drive. From the logs you have
attached:

I would recommend you scan the drive with a hard disk testing tool and
replace it if necessary. If you find that the disk is not in fact
dying, please feel free to reopen this bug report.
