Same problem here:
  - No LDAP
  - Dual-monitor setup (Intel HD graphics in i7-4770)
  - Usually two users are logged in graphically (VC 7 and 8)
  - Usually it starts happening for the second user (VC 8):
      - The box on the locked screen shows the user's name,
      - The box does not show the password entry field,
      - There are no icons shown in the top bar (i.e. no way to shutdown).
  - As long as the problem doesn't happen on the other VC, the session on the
    problematic VC can be recovered by:
      - Switching to the still working VC (which is in locked screen mode),
      - Clicking on the icon in the upper right corner, and selection "Switch
        Account...". Directly selecting the right account in the menu does NOT
        work, and merely switches to the problematic VC.
      - A new lightdm session appears. Log in with the account of the user
        whose session has problems.
      - The system switches to the problematic VC, and unlocks it.
  - Logging in on text VC1 and running "unity --replace" unlocks the
    problematic VC, but kills compiz.
  - Running "compiz --replace" in the graphical session restarts compiz, and
    locks the screen again, now showing a password entry box.
  - After unlocking the screen, everything works again, but all open windows
    have been been moved to the first desktop, so I have to untangle the desktop
    clutter manually.
