This is common, that the installers get confused by stale data on the disks. It could in fact be a bug, if the installer is supposed to clear the disks but fails to do so. But if we don't have a reproduce, it is hard to say for sure. Canonical: thank you for investigating this. If we find the sequence that led to this we will report back here.
