I've found some different behaviour that is related this morning. I started rhythmbox, browsed through the library and added some tracks to the playlist. I then clicked on the playlist (for the first time since starting the program) and the new tracks weren't added to the end of the playlist.
It might be that rhythmbox is intermittently scrambling the display the first time that the playlist is displayed, rather than the underlying storage being the problem.
