Fixed up the exclusion list so that it is clear where each bit is coming
from:

blacklist snd-mixer-oss
blacklist snd-pcm-oss

Also added new support to the module inclusion scripting to allow these
two OSS modules to be excluded, and use this to move them out to linux-
image-extra.
