I tracked it down to a corrupted VMWare image. For some weird reason
/dev/sda1 decided to have issues (a result of Ext4 in a VM? or perhaps
the host machine not writing out correctly?).

Either way, I believe the issue is well within the jurisdiction of
hardware/host drivers, and does not warrant further investigation. :)
