I'm seeing this bug, typically related with inserting a smartcard
reader, AND THEN inserting the smartcard:

[12030.538992] usb 1-1: new full-speed USB device number 6 using xhci_hcd
[12030.668391] usb 1-1: No LPM exit latency info found, disabling LPM.
[12030.676665] usb 1-1: New USB device found, idVendor=058f, idProduct=9540
[12030.676670] usb 1-1: New USB device strings: Mfr=1, Product=2, SerialNumber=0
[12030.676672] usb 1-1: Product: EMV Smartcard Reader

[12068.557399] xhci_hcd 0000:00:14.0: WARN Event TRB for slot 5 ep 6 with no

This is one reader for the Spanis DNIe (Electronic id card, used for a
lot of official stuf)
