Mauricio and I have investigated this, and found out that there is a code that checks for all FF's and might set the HBA offline and release queues, which might even crash the system, during our tests. The attached patch seems to fix all the problems we have seen, including when the adapter seems to have recovered, but can't do any IO. We have tested this on top of 4.0. Mauricio, can you build a package with this patch on top of Vivid's kernel, so this can be tested? Thanks.
