Also if it happens again, please include the following log files:
/home/phablet/.config/upstart/dbus.log
/home/phablet/.config/upstart/telephony-service-indicator.log
/home/phablet/.config/upstart/application-legacy-dialer-app-.log
and see if there are any files in /var/crash
