This bug is still alive in Trusty, apparently only on 3.16 kernels
(colleagues who are running 3.13 kernels do not have this issue):

$ cat /etc/issue

$ uname -a
Linux Depths 3.16.0-41-generic #57~14.04.1-Ubuntu SMP Thu Jun 18 18:01:13 UTC 
2015 x86_64 x86_64 x86_64 GNU/Linux

$ grep CONFIG_USB_OTG /boot/config-3.16.0-41-generic 
CONFIG_USB_OTG=y
# CONFIG_USB_OTG_WHITELIST is not set
# CONFIG_USB_OTG_BLACKLIST_HUB is not set
CONFIG_USB_OTG_FSM=m
