I should add that this is on a dell Vostro 1500 with a Sigmatel 9205 sound card, which uses the snd-hda-intel module for sound playback. I understand that there are already multiple bugs filed about the failure to mute the internal speakers that I mention in the last clause - the primary focus of the bug is the fact that the functionality of the internal speakers is influenced by whether external speakers are plugged in when the computer is booted.
