On this system, it seems that reboot could fix it temporarily. And it always
not working with cold boot.
This could be probably an issue with Bluetooth under USB 3.0 hub

However, running sudo apt-get dist-upgrade (3.16.0-39.53) could solve
this, I will mark this as Fix-released.
