It's hard to say when the problem has started to happen since it is not very reproducible. It happened multiple times, that's sure. But I have no idea how to trigger it.
I'll try the 4.2 unstable now (hopefully, my 14.04.3 system accepts it). Don't expect a "fixed upstream" confirmation before at least a month...
