Public bug reported:
during ripping down and freeing the aio request data structures we can
sometimes hit segfaults because we're still handling.
Solution is to:
1. cancel the signal handler
2. flag accounting is to stop (extra paranoid step)
3. put accounting stats on stack (extra paranoid step)
