Affects me too after upgrading to 15.04. When installing the packages, apt-get dist-upgrade terminated with an error processing mysql-server. I repeatead dist-upgrade, it installed mysql-server and another sql-related package I don't remember flawlessly. The 'report this' window popped up when logging in at the first reboot after that install of the mysql-server 5.6.25-0ubuntu0.15.04.1. I have no idea what exactly went wrong and afaik did not use or change mysql at all.
