I'm now able to reproduce this bug on demand.  It takes just under an
hour to reproduce.  I'm going to to try to reproduce a few more times to
make sure it's consistent.  If the reproduce is consistent, I'll list
all the steps necessary.

Basically I'm using the tiotest benchmark from tiobench.  The last
supported version of tiotest for Ubuntu was in Precise, but the Precise
version works fine on Wily, Vivid and Trusty.   If the reproducer is
consistent, I'll post a script with all the arguments to produce the
correct IO pattern,  if anyone else wants to test.

Now that I can reproduce this bug, I can run some of my own tests to try
and figure out what is happening.
