I had a similar problem, but it didn't happen from a dist-upgrade, but rather a regular upgrade.

Corollary question is whether this has to do with running an unsupported kernel, as the system I see this problem on is a Skylake CPU running 4.3.0-040300-generic vs stock 14.04.3 LTS on a supported CPU.
