bug from upstream commit 10b35b2b7485c342334a48cf199063eed8b8748e changes
behavior of PCI DMA on PowerNV/bare-metal platforms and can result in EEH when
adapter tries to read back buffer after DMA write.

Affects Wily and Xenial kernel streams. Based on code inspection, not
observation in running kernel
Machine Type = 8348 "Habanero LC"
---Steps to Reproduce---
Setup RAID volume on PMC-Sierra adapter (driver aacraid) and start a
test/exerciser doing disk reads on the volume. Pull a disk from the volume and
an EEH should result.

Fix is being reviewed and should get pushed upstream soon. Suggest pulling that
fix as soon as it is available. Patch is being reviewed on linuxppc-dev mailing
list, titled "powerpc/ioda: Set "read" permission when "write" is set".

The fix has been accepted and is now in "master" on linux.git.
