You can try this
{ USB_DEV_VER (0cf3, 3004, 00.02, 00.02), .driver_info = BTUSB_ATH3012 }
{ USB_DEV_VER (0cf3, 3004, 00.01, 00.01),.driver_info = BTUSB_QCA_ROME }
or something like that.
But I am not quite sure with the revision numbers. USB_DEV_VER uses
inclusive ranges.
