Confirmed that bug only happens on Ubuntu 16.04, not on previous Ubuntu
releases and not on Debian 8.3.

Machine has these OSes and can boot any at will, in case it helps:
Debian stable (8.3), Xubuntu 16.04, Xubuntu 15.04, Linux Mint 17.2
(based on Ubuntu 14.04), Fedora 23 XFCE Edition.
