It's not an issue in grub -- grub simply installs the bootloader in
EFI/ubuntu, not in EFI/boot. I don't know what happened to get the
system in this state; but to fix this we'll first need to know the
current state of boot variables -- could you please paste in the output
of the following command:

efibootmgr -v
